Our medical card evaluations are available for the New Mexico Medical Cannabis Program. This includes renewing medical card holders and new patients seeking a first time card. Review the list of State requirements for new patients. Renewals do not require additional medical records.

Eligibility Requirements
In order to be eligible for a medical card, each applicant will need the following:

Valid NM drivers license or NM ID.
(temporary ID acceptable) out of state, military IDs or passports are not acceptable. -
One of the listed qualifying medical conditions. Medical records must be provided at the time of your appointment. No medical records needed for PTSD or Anxiety Disorder evaluation.

Complete an in person or video appointment with Mindful Medicine Cannabis Cards.
